Becoming a Provider
Becoming a provider is a quick and easy process. We are happy to now provide many of the necessary resources online to help you get started.
Step 1: Download the provider profile and submit to TaWanda Dent. For assistance, call 317.205.8235.
Step 2: We will review the profile and schedule a Dawn Project orientation and provider presentation of services.
Step 3: Upon agreement to enter into a contract, download and submit the provider application and provider agreement.
Step 4: The Dawn Project reviews the provider agreement and application. Upon approval, you will receive service codes, contracted rates, invoicing instructions and effective date.
